The Epic1 Research Team will build your study and research tools in Epic to help improve patient safety and research workflow.  Please complete the Epic1 Research survey intake form to submit your request.  We will not activate your study in Epic until it is approved.  We can gather the appropriate information to begin research tool approval (ordering tools and BPA’s for research). Click below to access form.
